• If you are an existing, registered patient, you will need your PATIENT ID NUMBER which is printed on your letter or email, and your DATE OF BIRTH.
If you are booking an appointment for someone else, eg. your child, please fill in their details not your own.

• If you are an NHS patient and your last sight test was less than 2 years ago you may not be eligible for a free NHS test. Please call us to check your eligibility.
